Parents/Guardians of Native American Students -- If you have not ever completed a 506 Form for your student, please do so. You will login to your Sylogist portal and click Student Records Portal, click Update and then complete the 506 form, click the green checkmark and then click Submit to Site.
This only needs to be done once for a student, not yearly. So if you have completed the form before there is no need to do it again. If you are unsure, go ahead and complete it. Please do so by Friday, September 11. Thank you!
